Planetary fixes and improvements

Updates to the planetary SPH scheme:

  • Correctly implement the Balsara switch.
  • Port other changes over from the minimal-SPH scheme.
  • Introduced a fixed-entropy mode to planetary sph to help generate relaxed ICs.
